Project Example - PHP script to insert a BLOB into MySQL

The Task:

I need someone to debug a html/php script that inserts a blob into a mysql database. The webserver is apache, mysql v5.1, php 5.2. I will provide the html and php code that I can not get to work. The POST is failing and the data is not passed(via action on the form)
