Project Example - Order form

The Task:

We need someone to help us with forms. We have one project that needs to be done in PHP that is essentially a purchase order that tallies up each line item and totals the amounts. Then sends it to the company. It can have a separate button to calculate the amounts and another button to sendmail with all the customer info plus the order.
